Syracuse Jewish Family Service

Solutions Care Management assists individuals and families with common transitions and issues facing seniors or individuals with disabilities or chronic illness. Available services include assessment, care planning, information and referral both locally and nationally, service coordination, and communication between providers and between providers and family members; assistance with obtaining benefits forms; and help obtaining personal and home maintenance services. Ext. 231

Solutions also assists seniors and individuals with disabilities to manage household bills and paperwork, writing out checks, balancing financial statements, reading and writing personal correspondence, and completing applications, insurance forms and other official paperwork. This program is subsidized by a grant from the United Way of CNY.
A sliding fee scale keeps services affordable to most. Ext. 231
